Child employment / work permit application form

Who needs to apply for a work permit?

The employer should apply to the Child Employment and Performance Licensing Officer for a permit if the child will be working in the London Borough of Sutton.  A permit is for a specific job and cannot be transferred.

What you will need to complete an application:

  • Childs and parent details
  • Information on the role including proposed working days and times
  • A recent photograph of the child (within 6 months) (to be attached within)
  • A complete medical declaration form, filled out and signed by the parent/ guardian (to be attached within)
  • Risk assessment carried out for the employment of the child (to be attached within)

It is essential that this form is completed accurately (by parent / guardian) in the interests of the child’s safety. The Child Employment Officer must be notified immediately of any changes in relation to the child’s health.
