Child Performance Licence application form (Trial)

We are now looking to change to an online application form for performance licence applications, the following form is a trial for a selected few companies and subject to feedback and amendments, will be made available to everyone in June 2025.

Supporting documentation

Supporting documentation may be required depending on the nature of the application. Please make sure these are available to you whilst completing this new application. Required documents may include:

A copy of the child or young person’s birth certificate or satisfactory evidence of child’s age
Two passport sized photographs taken within the previous six months
Risk assessment – for location and relevant to the child
Child Protection Policy
A letter from the child’s school stating absence is permissible (if absence is required for the performance)

We must be satisfied that arrangement for the supervision and protection of the child are adequate and that the disruption to the child’s education is kept to a minimum.

Please download and complete the following medical declaration form to be complete and signed by the child’s parent/ guardian and uploaded when prompted within the application process.

For these purposes any performance taking place after midnight and before the earliest permitted hour counts as an extension to the previous day. E.g. if the child performs on Tuesday and then performs after midnight, in the early hours of Wednesday, that counts as one day (Tuesday).

The local authority may want assurance that there is a good reason why the child is asked to take part in a performance or activity at night.
